The following forms can be printed out and returned to SHW staff for registartion of participants

All form are kept confidentail and only used by SHW staff when needed, this helps SHW to provide athletes with a safe and secure environment.

All players must also register with USA Hockey, you can register online  by going to : USA Hockey Registration or by completing the Individual Membership Registartion form above and returning it to SHW staff. Players with not be able to participate if not regsiertered with USA Hockey as it provides insurance to the program.
